
ASSESSMENT
We welcome referrals for all our transport services from individuals or medical/community service agencies.
Under Government guidelines, all CHSP clients must first be assessed through the My Aged Care (MAC) platform. MAC can be contacted via their website www.myagedcare.gov.au or by calling 1800 200 422. Please call us if you require more information.
For NDIS participants, we welcome referrals from you, your carer or your NDIS Support Co-ordinators to Assist to access Community Social and Recreational Activities or for Transport. We will assess our capacity and provide a cost estimate and service agreement before facilitating a trip for a participant.
Some referrals have to be assessed by a staff member for eligibility. For example residents of hostels, or people receiving Community Aged Care Packages. These people are only eligible for the services we offer if we have vacancies, and if the client or agency agrees to pay the full cost for any service provided by RWCT.
Clients should be asked if they receive any other CHSP services to ascertain if their details have already been collected. Existing CIARR information can be sent via email.

NDIS
We can offer families a range of access and mobility services to enable you to participate in the community. Our expertise is the key enabler to get you to and from your supports. We assist participants to Access to Community Social and Recreation activities. We can also provide NDIS Travel and Transport to assist participants who receive the fortnightly Cash Travel Allowance from the NDIA, to access non-core or other supports.

WHO CAN USE THE SERVICES
People who are elderly, frail, or have an illness or disability that makes ordinary transport difficult. Our services are designed to assist people who don’t or can’t drive, cannot arrange transport through a friend or relative, can’t use public transport or afford ordinary transport.
